Middle suprarenal arteries

Definition
The middle suprarenal arteries are paired arterial branches that arise directly from the abdominal aorta and supply the adrenal (suprarenal) glands. They are one of the three primary arterial sources to the adrenal glands, the others being the superior suprarenal arteries (from the inferior phrenic arteries) and the inferior suprarenal arteries (from the renal arteries).

Anatomical Characteristics

Origin Typically arise laterally from the abdominal aorta at the level of the superior mesenteric artery, near the origin of the renal arteries (approximately the level of the first lumbar vertebra, L1).
Number Usually a single middle suprarenal artery on each side, though accessory or duplicated branches may be present.
Course After branching from the aorta, each artery passes posteriorly and laterally, running within the suspensory (fibrous) tissue of the adrenal gland. It reaches the medial aspect of the gland, where it divides into multiple small branches (rami) that penetrate the capsule and supply the adrenal cortex and medulla.
Relationships • Anteriorly: lies behind the peritoneum and the pancreas (right side) or the stomach (left side).
• Posteriorly: adjacent to the crus of the diaphragm and the vertebral bodies.
• Laterally: near the renal vein and renal artery on the corresponding side.
Branches The artery typically trifurcates or gives off several small arterial branches that anastomose with the superior and inferior suprarenal arteries, forming a rich vascular network within the adrenal gland.
Variations • Origin may be slightly cranial or caudal to the typical level.
• Occasionally arises as a common trunk with the renal artery or the inferior phrenic artery.
• Rarely, multiple middle suprarenal arteries may be present on one side.
Length Approximately 2–4 cm from its aortic origin to the adrenal gland, depending on individual anatomy and body habitus.
Diameter Typically 1–2 mm; may be larger in individuals with hypertensive or endocrine disorders that increase adrenal blood flow.

Embryology
During embryonic development, the adrenal gland receives a vascular supply from the lateral splanchnic (mesonephric) arteries, which are derivatives of the dorsal aorta. The middle suprarenal arteries represent the persistence of one of these early dorsal aortic branches. Their formation is closely linked to the migration of the adrenal primordium to its final retroperitoneal position.

Physiological Significance
The adrenal glands are highly vascularized to meet the metabolic demands of steroid hormone synthesis (cortisol, aldosterone, and adrenal androgens) and catecholamine production (epinephrine, norepinephrine). The middle suprarenal arteries contribute a substantial proportion of arterial inflow, particularly to the central and medial portions of the gland.

Clinical Relevance

  • Surgical considerations – During adrenalectomy (open or laparoscopic), identification and careful ligation of the middle suprarenal arteries are essential to control bleeding and to preserve surrounding structures such as the renal vessels and the inferior vena cava.
  • Radiological imaging – Contrast-enhanced CT or MR angiography can visualize the middle suprarenal arteries, aiding in the assessment of adrenal tumors (e.g., pheochromocytoma, adrenal cortical carcinoma) that may demonstrate hypervascularity.
  • Endovascular interventions – Embolization of the middle suprarenal arteries may be employed pre‑operatively to reduce intra‑operative blood loss for large, vascular adrenal lesions.
  • Pathology – Rarely, aneurysms or traumatic injuries involving the middle suprarenal arteries can cause retroperitoneal hemorrhage. Their proximity to the aorta makes them susceptible to iatrogenic injury during aortic aneurysm repair.

Anastomoses
The middle suprarenal arteries anastomose extensively with:

  • Superior suprarenal arteries (from the inferior phrenic arteries)
  • Inferior suprarenal arteries (from the renal arteries)

These connections create a collateral network that ensures continued perfusion if one source becomes compromised.
