Cuneiform bones

The cuneiform bones are three short, wedge‑shaped bones located in the midfoot, forming part of the tarsal region of the human skeleton. They are named for their shape (from the Latin cuneus, meaning “wedge”) and consist of the medial cuneiform, intermediate cuneiform, and lateral cuneiform.

Anatomical position

  • Medial cuneiform: The largest of the three; positioned medial to the intermediate cuneiform and lateral to the first metatarsal bone.
  • Intermediate cuneiform: Situated between the medial and lateral cuneiforms, articulating with the second metatarsal.
  • Lateral cuneiform: Lies lateral to the intermediate cuneiform and medial to the cuboid bone, articulating with the third metatarsal.

All three cuneiforms articulate anteriorly with the bases of the first, second, and third metatarsal bones, respectively, and posteriorly with the navicular bone. The lateral cuneiform also forms a joint with the cuboid bone.

Function
The cuneiform bones contribute to the stability and flexibility of the medial longitudinal arch of the foot. Their wedge shape allows for slight gliding movements that accommodate the foot’s adaptation to uneven surfaces during gait.

Clinical relevance

  • Fractures: Cuneiform fractures are uncommon but may occur in high‑energy trauma. They are diagnosed via radiography or computed tomography.
  • Lisfranc injury: Disruption of the tarsometatarsal joints, which involve the cuneiforms, can lead to Lisfranc ligament injury, requiring careful assessment and often surgical fixation.
  • Arthritis: Degenerative changes can affect the cuneiform‑metatarsal articulations, producing pain and limited motion.

Development
During embryogenesis, the cuneiform bones develop from mesenchymal condensations within the foot plate and ossify via endochondral ossification, typically beginning in early childhood.

Etymology
The term “cuneiform” derives from Latin cuneus (“wedge”) and ‑form (“in the shape of”), reflecting the bones’ characteristic wedge shape.
