- Tie‑breaking: Aggregate score determined the winner; if tied, the away goals rule applied, followed by extra time and penalty shoot‑out if necessary.

- 2017: Club Olimpia (Paraguay) – *Note: This is a mistaken entry; the inaugural champion was C.S.D. *—correct champion: Olimpia (Honduras) won the first edition.
- 2018: Herediano (Costa Rica) – First Costa Rican club to win the competition.
- 2019: Saprissa (Costa Rica) – Secured the title with a 4–2 aggregate win in the final.
- 2020: Olimpia (Honduras) – Became the first club to win the competition twice.
- 2021: Motagua (Honduras) – Won the final edition, defeating Alajuelense (Costa Rica) on aggregate.

Consequently, the CONCACAF League was discontinued after its 2021 edition.
